Position Summary
Under the direction of the Interim President and CEO, the Clinical Support Registered Nurse (CSRN) plays a vital role in
ensuring high-quality patient care and operational support across all clinical services. This role is designed to support
both leadership and frontline clinical teams. The CSRN collaborates closely with agency leadership, field and office staff,
referral sources, physicians, advanced practice providers, and other healthcare professionals to facilitate
comprehensive, patient-centered care.
Essential Qualifications
Education:
- Bachelor of Science in Nursing (BSN) preferred; Associate Degree in Nursing (ADN) from an accredited program
required.
Licensure:
- Current and valid Registered Nurse (RN) license in the state of practice.
Skills:
- Demonstrates strong clinical assessment and critical thinking skills.
- Proficient in electronic health record (EHR) systems and related medical software.
- Excellent communication and interpersonal skills to support effective collaboration with patients and interdisciplinary
teams.
- Ability to manage multiple priorities in a fast-paced healthcare environment.
- Compassionate and patient-centered approach, with a focus on enhancing the overall patient and family experience.
Work Experience:
- Minimum of 2 years of clinical nursing experience preferred, particularly in home health and hospice settings.
- Experience in a supportive or quality-focused clinical role is a plus.
Essential Functions
1. Reviews orders and plans of care for completeness and regulatory compliance; tracks trends and

regularly reports findings to the Leadership Team.
2. Performs audits of all Home Health Start of Care (SOC), Resumption of Care (ROC), Transfer, and
Discharge records for completeness, eligibility, and documentation standards; provides education
and feedback in alignment with Conditions of Participation (CoPs).
3. Assists with hospice audits, ensuring accuracy, program eligibility, and regulatory compliance,
while offering constructive feedback to staff.
4. Supports the Quality Manager in pulling and interpreting data from SHP and the EMR to inform
quality initiatives.
5. Assists with EMR training, implementation, and onboarding processes for clinical staff.
6. Provides ongoing computer and EMR support to clinicians in the field and office settings.
7. Serves as a backup to clinical leadership for both administrative and patient care functions as
needed.
8. Demonstrates strong organizational skills and exceptional customer service in all internal and
external interactions.
9. Collaborates with healthcare providers to develop and implement individualized patient care plans.
10. Assists with patient education to support health literacy and informed decision-making.
11. Supports the nursing team in executing clinical procedures, ensuring adherence to clinical
policies and safety standards.
12. Acts as a liaison between patients, families, and providers to ensure coordinated care and
address concerns effectively.
13. Participates in quality improvement initiatives to enhance outcomes and streamline workflows.
14. Provides emotional support to patients and families, fostering a compassionate and healing
environment.
15. Maintains accurate and complete patient records in accordance with HIPAA and regulatory
standards.
16. Engages in professional development to remain current on best practices and emerging
innovations.
17. Assists with inventory management of clinical supplies and equipment to ensure readiness for
patient care.

18. Adheres to agency policies, procedures, and ethical guidelines to support high standards of
nursing practice.
19. Performs all other duties as assigned.
